性與心：面對性功能障礙的創傷知情

 

好朋友李致樵泌尿科醫師，在粉專【泌尿科診療室李致樵醫師】分享許多平易近人的醫療知識。他曾經轉介有性功能障礙但是生理檢查沒有問題的診友來找我，我們從毒性壓力的角度來做一些身心調節。我整理了與性功能障礙與身心健康的資訊，希望能夠多一些對性功能障礙的了解。

 

憂鬱症:

與性慾減退、達到性高潮困難相關。沒有鬱症的人群中，有性功能障礙的比例為26%，而在未接受治療的鬱症患者中，比例增至45%，在接受藥物治療的患者中增至63%，與特定的藥物有關係(1)。

 

焦慮:

在有焦慮特質的女性，性的感覺可能引起恐懼，而不是愉悅。性疼痛在有焦慮疾患的女性中更普遍，比一般女性多10倍(2)。

 

創傷後壓力症候群：

患有PTSD男性的性功能與健康對照組相比，在性慾、高潮、活動和滿意度各方面都明顯較差。患者性功能障礙與SCL-90量表中憤怒-敵意子量表顯著相關，這說明情緒失調可能是性功能障礙的因素之一 (3)。在患有PTSD的女性退伍軍人中則很常出現陰道疼痛(4)。PTSD和性活動都涉及生理興奮，但健康的性功能還需要抑制恐懼和威脅網路，而PTSD過度活躍的威脅反應系統可能損害健康的性功能。正腎上腺素與皮質醇在PTSD中受到影響，也同樣影響了性功能。 (5)。

 

 

童年逆境經驗影響成年性功能：

曾經歷童年性虐待的成年人顯示出顯著較高水平的憂鬱、PTSD症狀、解離和性功能障礙。虐待期間暴力是一個重要預測因子(6)。在一項社交焦慮患者的研究中，36.3％的患者患有性功能障礙。而童年性虐待和共病憂鬱是社交焦慮患者患者的兩個強烈預測因子(7)。

在SMART課程導讀中，胡家琪博士提醒創傷中同時包含身、心的變化。性也是，需要同時兼顧身心，才能夠好好享受。
